Eastpoint Endodontics (In House Endodontics, 3397 N 1200 E, Suite 112, Lehi, UT 84043) respects your privacy. This policy explains what information we collect through our website, patient forms, email and text messages, and how we use and protect it.
As a dental practice, we are a covered entity under HIPAA. Your health information is also protected by our Notice of Privacy Practices, which describes your rights over your medical and dental records. Information we collect
- Contact details: name, address, phone and mobile number, email address.
- Patient information: date of birth, insurance details, medical and dental history, and information about your treatment, collected on our patient forms and during your care.
- Billing information: what you owe and pay. Card payments are processed by our payment processor; we do not store full card numbers.
- Messages: texts and emails you exchange with our office.
- Website information: what you submit through our contact or referral forms, and basic technical data such as your browser type and pages visited.
How we use it
- To schedule and remind you of appointments
- To provide, document and coordinate your dental care, including with your referring dentist
- To bill you and your insurance and process payments
- To send you paperwork, statements and messages about your care
- To answer your questions and requests
- To meet our legal obligations
We do not sell your personal information, and we do not use it for advertising by others.
Text messages (SMS)
If you agree to receive texts from Eastpoint Endodontics, on our patient forms or by telling our staff, we will text the mobile number you provide about your care.
- What we send: appointment reminders and confirmations, links to patient forms, billing notices, and replies from our staff to your questions. We do not send marketing texts.
- How often: message frequency varies with your appointments and conversations.
- Cost: message and data rates may apply.
- Opt out: reply STOP to any message to stop receiving texts. You’ll get one confirmation, then no more texts. Reply START to sign up again.
- Help: reply HELP, or call us at (801) 901-6095.
- Not required: agreeing to texts is not a condition of receiving care.
No sharing of mobile information. We do not sell, rent or share your mobile phone number or text-messaging consent with third parties or affiliates for their marketing or promotional purposes. Text-messaging opt-in data and consent are never shared with any third party, except the service providers that deliver our messages on our behalf.
Carriers are not liable for delayed or undelivered messages.
We email you about appointments, forms, statements and account access. You can ask us to stop non-essential emails at any time.
Who we share information with
We share information only as needed to care for you and run our practice: with your referring dentist and other providers involved in your care, your insurance company, and service providers that work for us, such as our practice-management software, payment processor and messaging services. These providers must protect your information and use it only for us, under HIPAA business associate agreements where required. We may also disclose information when the law requires it.
How we protect it
We use administrative, technical and physical safeguards, including encrypted connections, access controls and secure, U.S.-based data storage. No system is perfectly secure, but we work to protect your information and will notify you of a breach as the law requires.
Our website
Our website may use cookies and basic analytics to understand how visitors use it. You can block cookies in your browser settings. Please don’t send health information through our website contact form; call us instead.
Your choices
You may ask to update your contact information, change how we contact you, or opt out of texts and non-essential email. Your rights to see, copy or correct your health records are described in our Notice of Privacy Practices.
Children
We collect information about patients under 18 only from, or with the permission of, a parent or guardian.
Changes to this policy
We may update this policy. The date at the top shows when it last changed.
