THIS NOTICE DESCRIBES HOW MEDICAL AND DENTAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our responsibilities

Eastpoint Endodontics is required by law to keep your health information private, to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We will tell you promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us we can in writing, and you may change your mind at any time by letting us know in writing.

  • Payment: to bill and get paid by you, your insurance company or other payers.

  • Appointment reminders and communications: by phone, voicemail, text message, email or mail. You may ask us to contact you a different way.
  • Family and friends: with a person involved in your care or payment if you agree, or if you are unable to agree (for example, in an emergency) and we believe it is in your best interest.
  • Business associates: with companies that perform services for us, such as our practice-management software, claims clearinghouse, payment processor and messaging services. They must protect your information under written agreements.

Where Utah law gives your information greater protection than federal law, we follow Utah law.

Uses that require your written permission

We will never sell your information or use it for marketing without your written authorization. We do not conduct fundraising.

Your rights

You have the right to:

  • See or get a copy of your health and billing records, electronically or on paper. We will usually respond within 30 days and may charge a reasonable, cost-based fee.
  • Ask us to correct information you believe is incorrect or incomplete. We may say no, but we will explain why in writing within 60 days.
  • Request confidential communications, such as contacting you at a different phone number or address. We will agree to all reasonable requests.

  • Get a list of disclosures we have made in the six years before your request, other than for treatment, payment, operations and certain other purposes. One list per year is free.
  • Get a paper copy of this Notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you, such as a legal guardian or someone with medical power of attorney.
